Clinical Documentation Improvement: Using Metrics to Evaluate Your Program

The conversion from ICD-9 to ICD-10 is said not to affect a health care facility's case mix and or reimbursement in any appreciable way, not more than .5%. Is this really the case? There are particular diagnosis codes that are coded very differently in ICD-10, which will affect case mix and reimbursement. If your facility has a high volume of cardiology, orthopedics and/or obstetrics, documentation requirements for ICD-10 are very significantly more specific. Many payers will be doing backwards mapping from ICD-10 to ICD-9 to determine reimbursement. This may well not be consistent from payer to payer. As a health care facility, your analytics must be in place to anticipate appropriate reimbursement and to determine if the reimbursement you receive is correct. GEMS (General Equivalence Mappings) are useful, but they are approximate. If you use them to determine volumes and or reimbursement changes, you must be aware of how they should be used. In addition, your coders will experience as much as a 50% decrease in productivity in the first six months of ICD-10. If you have done your training of your coders, and more important, those who document in the record, you can expect to see productivity return to about 80% of ICD-9 productivity.
