It is important to understand that what you are putting together needs to represent the history of the patient relationship to maintain the record’s integrity. While it is a record of what decisions were made regarding the patient’s care and the patient’s payment of service, it is also your “legal testimony” as to what happened when the patient arrived and what actions were taken in the situation that a legal situation arises. The health record must be maintained in compliance with HIPAA regulations, accreditation, professional practice, and legal standards.
