Change-of-shift increases the risk to patient safety; learn effective patient hand-off to improve safety and accountability.
Confidently use information to improve skills in painting a picture of each patient during hand-off, from choosing the location to present the handoff to asking and answering question related to each patient during the hand-off which will adequately prepare the oncoming nurse to continue care for that patient with updated accurate information that minimizes risks of omission or missed opportunity to prevent decline in systems or cognition. To receive a certificate of successful completion, you are required to complete and return an evaluation form.
